This page explains how telehealth works through AACTIVATED Rx. Before your consultation, your Provider will also ask you to review and accept their own informed consent. If anything on this page differs from your Provider's consent, your Provider's consent controls.
Telehealth is not for emergencies. If you think you may have a medical emergency, call 911 or go to the nearest emergency room. Messages sent through the Services may not be read right away.
Your care is provided by independent, state-licensed health care providers licensed in the state where you are located at the time of your consultation. AACTIVATED Rx is not a medical practice and does not provide medical care. Your Provider uses independent professional judgment and may decide that a treatment is not right for you, ask for more information, labs or records, or recommend that you see a provider in person. No one at AACTIVATED Rx can promise or influence a prescription.
Telehealth means receiving care from a provider who is not in the same place as you, using online questionnaires, secure messaging, photos, phone or video. Your Provider may review your intake without a live visit where the law in your state allows it, or may require a phone or video visit.
Benefits include convenient access to care without travel. Possible risks include that information may be incomplete without an in-person exam; technical problems may delay care; and, despite security measures, electronic information could be accessed without authorization. Any medication has possible side effects. Your Provider will discuss risks, benefits and alternatives with you, and you may ask questions at any time.
You agree to give complete and accurate health information, to follow your Provider's instructions, to tell your Provider about side effects or changes in your health, and to keep your primary care provider informed of treatments you receive.
Your health information is protected by federal and state privacy laws, including HIPAA. Your Provider and Pharmacy will give you their Notice of Privacy Practices. See also our Privacy Policy.
Program prices, including what is included and what is billed separately, are shown at checkout. See our Terms and Conditions and Refund and Returns Policy.
Some states have additional telehealth requirements, which your Provider will follow and may present to you during your intake. You may request a copy of your medical records from your Provider.
By completing an intake through the Services, you confirm that you have read this page, that you understand you are consenting to receive care by telehealth from an independent Provider, that you may withdraw consent at any time by stopping use of the Services and telling us, and that you are at least 18 years old.
